Better ask the people at Ladbrokes, I have asked the same question but no answer, still waiting for an email reply from ladbrokes.

im pretty sure the cashout requirement was something stupid.. but i could be mistaken that for another site.. but im almost 98% it was them...

Maybe it has nothing to do with it if your request was done a while ago, but Ladbrokes have currently frozen any bonus giveout temporarily due to security issues.
